VNA Care Choices—An Innovative Approach to Care



Many seniors and family members of individuals who suffer from life-limiting illnesses must choose between the support services provided through hospice care or curative treatment. Fewer than half of eligible Medicare beneficiaries use hospice care and most only for a short period of time. Under current Medicare payment rules, individuals are not able to receive both palliative and curative treatment concurrently.

To bridge the gap, the Medicare Care Choices Model provides at-home palliative care offering additional support services in the home while continuing curative treatment. The program gives patients and families greater control in care decisions, access to a dedicated care team to manage pain and other symptoms along with spiritual, social and emotional support. This new approach offers the following:

  • 24/7 access for symptom management
  • Nurse Navigator helping coordinate care
  • Caregiver support
  • Social Work, Chaplain, Home Health Aide
  • Flexibility in treatment options

VNA is providing this innovative pilot program in North Texas with VNA Care Choices. Since 1934, VNA has been a pioneer in innovative health care concepts for North Texans. As the first hospice agency in Texas, VNA is honored to be a part of this pilot program.
